This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment Program Approval award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application Former EDRP participants ineligible to apply.
Duties
Responsible and accountable for individual nursing practice within the PACT Outpatient setting. Determines appropriate delegation of tasks to ensure optimal patient care. Provides gender-specific direct care and care coordination for Veterans in the Primary Care Department. Manages assigned patient panels and uses registries to ensure appropriate evaluation, access, and follow-up. Delivers health promotion, health education, and wellness coaching. Supports disease prevention and chronic care management initiatives. Focuses on professional growth and development in outpatient PACT nursing practice with increasing self-direction. Formulates plans of care based on nursing assessments. Seeks guidance to improve care provided to patients and families. Delegates elements of care based on patient needs, condition, and safety considerations. Applies primary care nursing principles and theory with limited use of critical thinking skills. Requirements
