Summary

The Women's Health Clinical Navigator is responsible for providing competent, evidence-based care to assigned patients. They provide patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing.

Duties

THIS IS NOT A REMOTE POSITION Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinate care with multiple providers across all levels and sites of care. Assure services interfacing within the Veterans Affairs (VA) and community programs provide continuity of care to Veterans. Coordinate and facilitate WH care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. Interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Requirements

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
