Summary
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self management after discharge, and supporting factors that impact customer satisfaction.
Duties
Major Duties: Works in collaboration with primary care clinics and other areas to maintain access of data sources and reports. Travel to assigned clinics (Rome, Blairsville, and Pickens) to train, troubleshoot, and conduct environment of care rounds on telehealth equipment. Travel to other clinics as needed to provide telehealth coverage. Provides professional assessment, coordination and planning of multiple health care services; acts on behalf of the veteran to assure that necessary clinical services are received, and that progress is being made. Facilitates Veterans' access to care, including those Veterans in rural and highly rural areas. Prepares patient for telehealth visit (connection, security, room set up, expectations). Uses telephone, videoconferencing, or other web-based technologies. Serves as a Tele presenter in the clinical setting and facilitates comprehensive exams/assessment under provider guidance (i.e. listen to heart, abdominal, lung sounds, etc.) Reviews the data delivered via the remote patient monitoring, then collaborate with the patient's PCP, and finally have a videoconferencing session with the patient to coordinate care. Provides training sessions related to Telehealth Initiatives Provides clinical support working within their scope to support and conduct telehealth related activities. Collaborates with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal and community-bases services involved in providing care to the patient) in developing the patient-driven care plan for life. Familiarity with the Conditions of Participation (COP), as this focuses on the continuous quality improvement model that includes all Telehealth and Connected Health programs Enrolls/manages patients, verify provider orders and patient consent, assists patient with participation Reviews test results and provide education Educates patients synchronously and asynchronously Serves as champions or advocates for telehealth within their organizations regardless of location. Requirements
