Summary
The Homeless Patient Aligned Care Team (HPACT) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
Duties
Major Duties include, but are not limited to: Provides direct Veteran care and care coordination, as it pertains, to homeless Veterans and their care within the Health Care for Homeless Veterans Outreach Clinic. Manages assigned panel, utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. The HPACT RN provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Formulates a plan of care based on nursing assessments. Conducts and participates in outreach activities, coordinate any travel scheduled with rural clinical sites, community partners, at Veterans Affairs (VA) Stand-Downs, and for any other homeless special events. Functions as point of contact with Veteran's Service Organizations (VSO), etc., to recruit and arrange for new homeless patient appointments. Recognizes the signs of potential or active disruptive behavior and quickly intervenes with appropriate therapeutic interventions. Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients. Manages communications and facilitates safe transition of patients between the PACT sites of care and other health care settings. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Requirements
U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Preferred Experience: Experience working on a Primary Care Team, serving veterans, and/or serving the homeless population.
