Summary

The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN), GS-7 in Primary Care (PC), provides skilled and individualized practical nursing care to all levels of patients including most complex patients who are acutely and/or chronically ill in the outpatient care setting. The PACT Team Licensed Practical Nurse at the GS-7 level has the knowledge and skills necessary for a concentrated effort and self-direction in carrying out the most complex practical nursing assignments.

Promote veterans' functional independence through daily-living and mobility activities. Support frontline patient check-in, clinic flow, patient education, and care preparation. Collaborate with PACT Core and Expanded Team to coordinate care. Serve as essential personnel during emergencies. Perform phlebotomy. FUNCTIONS OF PRACTICE Use patient data to contribute to veterans' health plans and implement interventions. Follow program regulations and meet nursing program requirements. Develop and update plans of care and obtain RN/MD review. Prepare and administer medications and perform treatments per policy. Observe and document changes in health status and report deviations promptly. Take action based on observations, care reports, and provider communication. Report and record patient information and modify care plans accordingly. Perform treatments, procedures, and reprocess reusable medical equipment. Support monitoring of unlicensed staff to ensure safe care. PROFESSIONAL DEVELOPMENT / EDUCATION Seek educational opportunities and participate in committees and task groups. Complete mandatory reviews and pursue personal growth. Participate in nursing in-services. Assist with orientation of nursing staff, students, non-medical staff, and veterans/families. SAFETY Respond appropriately in emergency situations. Promote a safe, therapeutic environment. Report unsafe conditions or malfunctioning equipment and initiate corrective actions. PRIMARY CARE CLINIC LPN DUTIES Provide nursing care to patients with chronic, acute, or terminal conditions with minimal RN/provider supervision. Independently triage patients and collect physical/psychological data. Administer and document medications and treatments safely. Initiate emergency actions for cardiac/respiratory arrest, seizures, or behavioral crises; coordinate with mental health. Provide patient/family health education and conduct one-on-one or group teaching. Assist providers with diagnostic/therapeutic procedures (biopsies, joint aspirations, phlebotomy, pap smears, pelvic exams, minor surgeries). CUSTOMER SERVICE Practice positive customer service techniques and maintain courteous interactions. Respect privacy, dignity, personal values, culture, and belief systems. Protect patient rights and participate in resolving ethical issues. Follow up on abnormal labs and support performance measure goals. Coordinate smooth inpatient-to-outpatient transitions and ensure timely access to care. MEDICATION ADMINISTRATION Prepare, administer, and document medications through permitted routes. Provide medication education to patients/families. Operate flu clinics and administer immunizations and injections. ENVIRONMENTAL MANAGEMENT Maintain a safe and therapeutic environment and respond appropriately to hazards. Report malfunctioning equipment or supply issues and request corrective action. Identify infection control issues and perform appropriate interventions. Assist with clinic setup, maintain supplies, and manage patient education materials. SUPPORT OF ORGANIZATIONAL GOALS / LEADERSHIP Support VA vision, mission, values, and goals through team participation. Serve as preceptor for new or less experienced staff. Seek educational opportunities and assist others in advancing practice. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Requirements

You must be a U.S. Citizen to apply for this job.
